"Active members of CASE are eligible to participate in a Group Life Insurance Plan underwritten by The Hartford AND/OR a Long Term Disability Insurance Plan underwritten by The Standard. The summary of benefits and the enrollment forms are here for both. There is going to be an OPEN ENROLLMENT period for Long Term Disability Insurance for all members who did not apply during the month of September, 2005. If you wish to read the Long Term Disability Contract, please request via email. Anyone who applies before January 30, 2006 will be added to the group policy effective February 1, 2006. Applications must be received in the NBIS office by mail by January 30 to be accepted and no other OPEN ENROLLMENT period will be offered."

New CASE members will have a short window to enroll and will have their own "Open Enrollment" period.
